1. Who will be the primary caregiver for this Cavalier and your Occupation/ hours of work per day.
Please list all people living in the household. Including name, relationship, and age.

2. Do you have a fenced yard? What type of fencing?

3. How will you exercise your puppy? How active is your household? Are you interested in doing any activities with your dog? Therapy? Rally? Show?

12. Generally a healthy and long lived breed, Cav's genetic background can have some hereditary defects. Have you researched their health concerns?
